Patient Information

As a chiropractic patient, it is important you receive accurate, relevant and clear information to enable you to make information decisions about your care.  The following outlines this information and Living Well Chiropractic requests your consent to relevant elements of your care.  If at any time you have questions about your care or wish to update your consents, the Chiropractor will be happy to assist you.  Please read the information below.

Cancellation and 'Did Not Attend' Policy

If you are unable to attend your appointment, please aim to provide at least 24 hours notice.  This means your appointment can be made available to another patient.  If you do not make contact to rearrange or cancel your appointment you will be invoiced and requiried to pay the full amount of the appointment.

Consent for Sharing of Information with Appropriate Health Care Professional, Including your General Practitioner

If you have been referred for chiropractic care by another health care professional, we would like to keep them up to date with your progress.  In addition, as a standard level of communication, we would like to provide your GP with a copy of your symptoms, assessment, report of findings and update them with your progress.

Consent to be Examined/Assessed

In order to fully understand what is causing your symptoms, the Chiropractor will be required to conduct a physical examination.  The Chiropractor would like to assess you at your consultation appointment and at follow up appointments.

Whenever possible, the Chiropractor will keep you fully clothed when performing examinations.  The Chiropractor may wish to perform tests that require you to remove clothing.  If this is the case, the Chiropractor will ask for your verbal consent, provide you with a gown to cover yourself and provide you with privacy to undress and redress.  If you do not wish to remove any clothing, then the Chiropractor will find an alternative way to gather the information desired.

Prior to performing each test, the Chiropractor will explain what is involved before each test is conducted.  If you are not comfortable with any part of the assessment, please make the Chiropractor aware.  Some of the assessment may include tests known as 'provocative tests' which may aggravate the symptoms.  The intention of the assessment is always to gain insight into what is causing your sysmptoms so that appropriate care be provided.  If symptoms are aggravated, the intention is for the aggravation to only last a short amount of time/the amount of time that particular provocative test is performed.

Consent to be Adjusted

At each follow up appointment, the Chiropractor will review your progress.  Before any adjustments are performed the Chiropractor will ensure Chiropractic remains appropriate and ensure you are happy to be adjusted.  Please make the Chiropractor aware, should you not wish to be adjusted.

It is a rarity the Chiropractor will require you to remove any clothing in order to adjust you.  Though should the Chiroprctor think this is required, they will always request your verbal consent beforehand and provide you with same level of privacy outlined in the examination consent statement.
